By entering Bespoke Longevity & Aesthetic Medicine (“Bespoke”, “the Clinic” or “we”), patients, visitors and contractors agree to comply with the following Conditions of Entry.

1.CCTV & Security

CCTV cameras are in operation in designated areas of the Clinic for security, safety and the protection of patients, visitors, staff and property.

There are no CCTV cameras located in treatment rooms or consultation rooms.

By entering the premises, you acknowledge that CCTV recording may take place in designated areas. Entry to the Clinic is subject to these conditions.

2.Respectful Behaviour

Bespoke is committed to providing a safe, professional and respectful environment for patients, visitors and staff.

Aggressive, threatening, abusive, intimidating, discriminatory or inappropriate behaviour towards staff, practitioners, patients or visitors will not be tolerated.

The Clinic reserves the right to ask any person engaging in inappropriate behaviour to leave the premises and, where appropriate, to restrict future access to the Clinic.

3.Appointments

Appointments are subject to availability and confirmation by the Clinic.

Patients are requested to arrive on time for their appointment. Late arrival may reduce the time available for the consultation or treatment and, where clinically or operationally necessary, the appointment may need to be rescheduled.

Cancellation and rescheduling fees may apply in accordance with the Clinic’s current cancellation policy.

4.Consultation & Clinical Assessment

All treatments and clinical services are subject to an appropriate consultation and assessment by a suitably qualified practitioner where required.

A consultation does not guarantee that a particular treatment, procedure or product will be recommended or provided. The practitioner will determine whether a treatment is clinically appropriate based on the individual circumstances of the patient.

The Clinic may decline or postpone treatment where it is considered clinically inappropriate, unsafe or unsuitable.

Pricing and products and brands are unable to be discussed over the phone or on email our social media platforms, a consultation is required with a medical practitioner as per the AHPRA regulations and TGA guidelines

5.Quotes, Treatment Plans & Costs

Where applicable, patients will be provided with information regarding recommended services, treatment plans and associated fees.

Quotes are provided based on the information available at the time and may change where the scope of services changes or additional services are requested.

All invoices and agreed fees are payable in full in accordance with the Clinic’s payment terms.

Patients are responsible for understanding the costs associated with their chosen services before proceeding. Please speak with our team if you require clarification regarding fees or payment arrangements.

7.Treatment Outcomes

Individual treatment outcomes vary between patients. No particular cosmetic, aesthetic, health or longevity outcome can be guaranteed.

Patients acknowledge that results may be influenced by individual circumstances, adherence to practitioner recommendations, lifestyle factors and other variables.

8.Patient Information & Consent forms

Patients are responsible for providing accurate and complete information relevant to their consultation or treatment, including medical history, medications, allergies, previous treatments and any changes to their health.

Patients should notify their practitioner of any relevant changes before treatment.

A consent form is required to be completed prior to treatment taking place

9.Privacy & Confidentiality

Bespoke respects patient privacy and handles personal and health information in accordance with applicable privacy and healthcare requirements.

Patients must respect the privacy of other patients and staff while attending the Clinic. Recording, photographing or filming other patients or staff without appropriate consent is not permitted.

10.Personal Belongings

Patients and visitors are responsible for their personal belongings while attending the Clinic.

Bespoke accepts no responsibility for loss, theft or damage to personal property, except where required by law.

11.Mobile Phones & Recording

Mobile phones should be placed on silent while attending consultations and treatments.

Photography, audio recording, video recording or livestreaming within the Clinic requires prior consent from the Clinic and must not compromise the privacy of other patients, visitors or staff.

12.Health & Safety

Patients must follow reasonable instructions provided by Clinic staff and practitioners relating to health, safety and infection control.

Any person who is unwell or experiencing symptoms that may present a risk to others should contact the Clinic before attending an appointment and may be required to wear a face mask.

13.Medication & Products

Where treatments, medications or products are recommended or supplied, patients must follow the directions and instructions provided by their treating practitioner.

Patients should raise any concerns, adverse reactions or unexpected outcomes with the Clinic or their treating practitioner as soon as appropriate.

14.Right to Refuse or Restrict Entry

Bespoke reserves the right to refuse or restrict entry where reasonably necessary for the safety, security, privacy or wellbeing of patients, staff, practitioners or visitors, or where a person has breached these Conditions of Entry.

This does not affect any rights or obligations arising under applicable Australian law.

15.Complaints & Concerns

We value patient feedback and encourage patients to raise any concerns directly with the Clinic.

Complaints will be treated seriously and reviewed in accordance with the Clinic’s applicable complaints processes.
